Preface
Despite the passage of seven years since the publication of Core Topics in Cardiac Anaesthesia, the fundamental principles of cardiac anesthesia remain
unchanged. In contrast, the clinical landscape has
undergone significant change – the resurgence of primary percutaneous intervention in acute coronary
syndromes, new developments in electrophysiology,
use of percutaneous devices in patients previously
considered inoperable, a reawakening of interest in
extracorporeal support in the wake of global influenza
pandemics, and the withdrawal of aprotinin.
Although primarily aimed at residents and fellows
in anesthesia, Core Topics in Cardiac Anaesthesia
proved popular with residents and fellows in cardiac
surgery, clinical perfusionists and critical care nurses.
In preparing the second edition we have carefully considered the advice of our readers and reviewers. Their
suggestions were unambiguous; explain complex topics
in an easy to digest and a readily accessible manner, use
figures and tables in preference to text, and avoid all
unnecessary repetition and bias. Above all they were
anxious for us to retain the first edition’s brevity, clarity
and portability. Accordingly, our instructions to contributing authors and editorial aims were unchanged;
produce a concise yet comprehensive overview of the
subject emphasizing pathophysiology, basic scientific
principles and the key elements of practice.
The title, selection of contributors and style of
content of the second edition have been carefully
adjusted to give it a greater international appeal. The
pharmacology section has been changed to present a
large volume of data in an accessible and more
appealing format. There are new chapters on right
heart valves, pulmonary vascular disease, cardiac
tumors and cardiac trauma. Acknowledging the
changing role of the cardiothoracic anesthesiologist,
we have added chapters on catheter laboratory and
hybrid operating room procedures, and acute coronary syndromes. Extracorporeal support is now discussed in a separate chapter and antimicrobial
prophylaxis, resuscitation and perioperative transesophageal echocardiography (TEE) have been updated
to reflect changes in international guidelines. Lastly,
we present a eulogy to aprotinin.
Certification and accreditation in perioperative
TEE has now become the de facto “Board” qualification for cardiothoracic anesthesiologists in many
countries. Unfortunately, this merely assures the quality of echocardiography instead of the wide range of
knowledge and competencies expected of a well
rounded specialist in cardiac anesthesia. Because there
are already several excellent TEE books, we have
strenuously resisted the temptation to turn our second
edition into a “TEE manual”.

the publication of this volume possible; our international panel of contributors for taking the time to
share their knowledge and expertise; Deborah Russell
and Joanna Chamberlin of Cambridge University
Press for their encouragement, advice, generosity
and seemingly inexhaustible patience; and our Specialist Registrars for their advice and proofreading.
